TC2 wants to be able to identify “rising risk” beneficiaries to decrease ED & inpatient utilization, and reduce the frequency of readmissions. We have not found an appropriate off-the-shelf solution that works in our environment and are therefore looking for your help in co-developing a novel model. The Medical Center of Peach County (MCPC), Navicent Health is a critical access hospital that serves the needs of rural neighboring communities.
Asthma is one of the primary diagnosis at MCPC ER for children. Community research in Fort Valley has revealed a knowledge deficit about asthma and asthma triggers. We are looking to bridge the knowledge gap through an approach that would benefit the communities we serve. Do you have ideas to help communities breathe better?
