(Middle GA ACO with CMS)

TC2 is an ACO serving Medicare beneficiaries in Middle GA.
TC2 wants to be able to identify “rising risk” beneficiaries to decrease ED & inpatient utilization, and reduce the frequency of readmissions. We have not found an appropriate off-the-shelf solution that works in our environment and are therefore looking for your help in co-developing a novel model. Do you have ideas on how to make this work?
